
The Integrated Referral and Intake System (IRIS) is a web-based tool to help member organizations connect the families they serve to the right resources in their community.
About the IRIS Referral Network
In August 2025, PAASSS launched an online referral system called Integrated Referral and Intake System (IRIS). There are currently more than forty programs that can send/receive client referrals. Previously, there was no way to track if a referral was acted upon. Most agencies were only able to give out resource guides or phone numbers and might never know if a client received the services they needed.
The IRIS system is HIPAA compliant and tracks referrals to see how many referrals are made, how many referrals are accepted, and how many are rejected. With IRIS, a partner agency gets an email to alert them that a referral has been made. PAASSS has established a community standard requiring partner members to respond to a referral within five business days. The system also allows for the referring organization to receive notifications about their client’s referral process.
In Grundy and Kendall Counties, our IRIS system is also tracking zip code data to find out where the greatest needs for services are located and what types of services are needed. Referral data is reviewed quarterly in order to improve the system, to determine which sectors need more representation in the network, and to set goals and review progress toward community outreach and impact. The IRIS system is being used across Illinois, Kansas, Utah, Missouri, Iowa and the Boston area.

What is the IRIS Data Dashboard?
The IRIS Data Dashboard is an interactive, community-level resource built for our IRIS network (serving Grundy & Kendall Counties). It offers a transparent, real-time look at how our partner organizations — across early childhood, family support, behavioral health, education, housing, and more — are engaging with families through referrals and services. The dashboard allows stakeholders, community members, and funders to see aggregated, privacy-protected data about partner engagement, referral volume, service access, capacity, and trends over time.
By making this information public, the dashboard helps to:
- highlight where services are being accessed,
- identify patterns or gaps in community need,
- support data-driven decision-making and collaboration among partners, and
- allow the public to better understand the scope and impact of the IRIS network in our community.

Devan Gagliardo, IRIS System Manager
Devan serves as Program Director at the Community Foundation of Grundy County, a role she has held for 14 years. In her work, Devan leads strategic grantmaking, fosters partnerships, and strengthens local networks that support residents and families. She collaborates closely with the Behavioral Health Alliance, Grundy County Interagency Council, Grundy Partnership for Children, and 100+ Women Who Care of Grundy County. Devan also leads a Youth Philanthropy program across four local high schools, helping students learn the impact of community giving and leadership. In addition, she serves on the Justice Assistance Board.

Ryan Carrie, L.S.W. IRIS Data Manager
Ryan has dedicated over seven years to serving individuals and families through Community Action in Kendall County. He began his career at the Kendall County Health Department as an intern with Community Action Services, where his passion for helping others and strengthening community connections quickly became evident.
Currently, Ryan serves as the Assistant Director of Community Action Services at the Kendall County Health Department and as the IRIS Data Manager. In these roles, he works to ensure that programs operate efficiently, data is used meaningfully, and resources reach those who need them most.
With a strong commitment to collaboration, Ryan facilitates the monthly Kendall County Interagency Council meetings, bringing together local partners to identify community needs, share resources, and strengthen service delivery across the county. He is especially passionate about identifying gaps in support systems and connecting individuals and families to the services that can help them thrive.
